As a courtesy, we will file your insurance. Please bring
one of the above forms to your appointment, along
with your insurance card and insurance carrier's
complete address.
You must pay your deductible and any co-pay in full
at each office visit. Memphis Neurology accepts cash, check, Mastercard,
Visa, AMEX and Discover Card. If you are unable to pay in
full, let us know. We can set up a monthly payment
plan.
If your insurance plan requires a
referral, please bring a current referral with you on
the date of visit.
If you do not have the required forms, your
appointment will be rescheduled.
