Unless you tell us otherwise, a medical report will be sent to the referring physician. If you ask, we will also send a report to your child's school.

Because we are seeing more patients and the demand for letters and forms has risen, we are charging $10 for one-page letters for school, work, insurance companies, attorneys, etc. Family leave forms for work and disability or other detailed letters are $20.
